Chronological list of CART-IRL team and driver switches.

I would like it if someone made a chronological list of when drivers and teams moved from CART to the IRL, or in a few cases, when they moved from IRL to CART during the split. Can someone do that?

This may or may not be complete, since I'm just going off the Wikipedia pages. I'm also listing teams that ran both series in one year. Since the IRL ran two seasons in 1996 (1996 and 96-97), the years are listed as such below. Teams in italics were CART regulars who also ran Indianapolis.
