So far my list has four cars on it.

Life F190 - Yeah Slow and unreliable doesn't even begin to start to describe this car.
Andrea Moda S921 - We all know how comically bad this car was.
BRM P351 - car ran in to so many issues in the 92 Le Mans that the ACO basically made it Start and Park* (This isn't quite as bad as it sounds though)
Ford Mustang GTP - Car won its first race then massive reliability issues afterwards. Enough issues to warrant it's inclusion on the list.  
Lotus Indy Car Engine - Not a car but I'll let it count. (Suggested on facebook)
Lamborghini 291 - one of the worst factory efforts in racing history.




*The BRM ran into so many issues during the practice that only one driver (Wayne Taylor) out of the three entered were able to get any time during practice. Because of this poor Wayne was the only driver allowed to run it. ACO Rules also dictated that no driver could run more then 14 hours solo, so in theory the BRM couldn't go any further then 14 hours before absolutely having to park it. The car did manage to last 30 laps before being taken out in a pit fire.

Nice thread...racing has had some interesting failures throughout its history.

* The Ford GT 40 was one of the greatest race cars of all time but the Ford Mk. I, which was introduced at the 1964 Le Mans. Despite a well funded effort and a top notch driver lineup (Richie Ginther, Masten Gregory, Bruce McLaren, Phil Hill, Jo Schlesser, and Richard Attwood) the car was a disaster. After upgrades the cars improved dramatically, but in its original form the Ford GT wasn't great.
* In 2011, Aston Martin replaced its quite successful Lola Aston Martins with the AMR-One, a car built entirely by Aston Martin and its racing parent company Prodrive. The car had a disappointing showing at Paul Ricard and the Le Mans Test Day was a disaster. They skipped the Spa 1000 and went straight to Le Mans, entering two cars. The #007 spent four hours in and out of the pits and the #009 did two flying laps at the start before having a massive mechanical failure. The car's story didn't end there though. The chassis evolved into the Pescarolo 03 and provided the basis for the DeltaWing (Aston Martin retains the other car as a show car). In 2012, AMR went back to GT racing.
* Chapparal 2H was the least successful of the long range of the radically designed (usually GM engined) Texas-built sports cars. At one point featuring a comically huge wing above the cockpit, the car never saw the success of some of the other Chapparals.
